    A. Certificate of Criminal Record

    1. Eligibility

·   Macao SAR Resident or person who has formerly resided in Macao SAR; and

·   Aged 16 or over.

 

    2. Application Procedures and Documents:

i)        Macao Resident Identity Card (BIR) holders

(a)       Application made in person

l Please present the original BIR

(b)       Application made by applicant’s direct relative (i.e. parents, spouse or children aged 18 or over)

 

Please submit the following:

 

l Photocopy of the applicant's Macao Resident Identity Card; 

l Original identity document of the applicant's direct relative;

l Documentary proof of the kinship (if the relevant data is not recorded in Identification Department);

l Documentary proof of why the applicant cannot apply in person.

l A full set of the applicant's ten fingerprints.

 

(c)  Application made by authorized person

Please submit the following:

·    Proxy or letter of authorization（Samples can be obtained at DSI）

·    Photocopy of the applicant's Macao Resident Identity Card;

·    Original identity document of the authorized person.

·    A full set of the applicant's ten fingerprints.

 

ii)  Non Macao Resident Identity Card (Non-BIR) holders

 

(a)       Application made in person

Please submit the following:

 

·    Documentary proof of the connection between the applicant and Macao, such as Residence Permit, Identity Permit of Non-Resident Worker, proof of schooling or proof of application for residing in Macao (e.g. a notice of Residence Permit issued by Immigration Department);

·    Valid travel document/Passport issued by the country where the applicant currently resides or in where the applicant is a national, or a valid document for entering Macao such as One-Way Exit Permit of People’s Republic of China.

(b)       Application made by applicant’s direct relative (i.e. parents, spouse or children aged 18 or over)

 

Please submit the following:

·   Photocopy of the documentary proof of the connection between the applicant and Macao;

·    Photocopy of the valid travel document/passport issued by the country where the applicant currently resides or in where the applicant is a national, or photocopy of the valid document for entering Macao;

·    Original identity document of the applicant's direct relative;

·    Documentary proof of the kinship (e.g. birth certificate or marriage certificate)

·    Documentary proof of why the applicant cannot apply in person.

·    A full set of the applicant’s ten fingerprints.

 

(c)  Application made by authorized person

Please submit the following:

·    Proxy or letter of authorization（Samples can be obtained at DSI）

·   Photocopy of the documentary proof of the connection between the applicant and Macao;

·   Photocopy of the valid travel document/passport issued by the country where the applicant currently resides or in where the applicant is a national, or photocopy of the valid document for entering Macao;

·    Original identity document of the authorized person;

·    A full set of the applicant’s ten fingerprints.

5. Overseas Applications

 

Applicants residing abroad may apply by mail.

Applicants should apply by completing the application form or by letter. The letter should contain: name of applicant, parents’ name, date and place of birth, nationality, marital status, usage of the certificate, contact information and applicant’s signature with right index finger imprinted next to the signature. (For first time application, please imprint all ten fingers.)

 

Ø      Please submit the following documents:

·         Proof of residence in Macao, such as identity documents issued in Macao, proof of schooling or a notice of Residence Permit issued by Immigration Department;

·         Photocopy of valid Travel document/Passport issued by the country where the applicant currently resides or in where  the applicant is a national.

·         To confirm the names of parents printed on the certificate, please submit a photocopy of the relevant documents if the data is not recorded in Identification Services Bureau (i.e. birth certificate).

6. Term of Validity

    B. Certificate of Special Record for Minors

 

    1. The content of Certificate of Special Record for Minors:

·    Educational measures imposed on the minor over the age of 12 and under the age of 16;

·    The above record will be cancelled after applicant reaches the age of 21.

    2. Eligibility

·    Resident of the Macao SAR or has formerly resided in the Macao SAR;

·    over the age of 12 and under the age of 21;

·    if the applicant is under the age of 16, application must be made by his/her legal representative;

·    if the applicant is or over the age of 16, application could be made in person, or by his/her parents, spouse or an authorized person.

 

    3. Application Procedures and Documents:

i)    Macao Resident Identity Card (BIR) holders:

Ø If the applicant is under the age of 16, application must be submitted by his/her legal representative (parents or guardian);

·    Photocopy of the applicant’s Macao Resident Identity Card; 

·    Original identity document of the applicant’s legal representative. 

·    If first time applying, applicants are required to also submit the 10 fingers imprint. 

 

 

Ø If the applicant is or over the age of 16

 

a)     Application made in person

Please present the original BIR

b)     Applications made by applicants’ direct relative (i.e. parents or spouse)

Please submit the following:

·    Photocopy of the applicant's Macao Resident Identity Card; 

·    Original identity document of the applicant's direct relative;

    C. Certificate of Residence for Foreign Use

 

Ø Macao Resident Identity Card (BIR) holder

l        Photocopy of BIR

l        A completed application form.  If the applicant is under the age of 18, one of the parents must sign on the form.

 

Fees:

·    MOP$30.00

 

Collection Date:

·    Three working days upon receipt of all necessary documents.

 

OBS:

    D. Certificate of Residence for Obtaining BIR

 

                Starting from May 24, 1999, the Certificate of Residence must be submitted by the following first-time BIR applicants:

·    BICN holders

·    One Way Exit Permit holders

                        Please apply the aforesaid certificate at the Immigration Department.

　

　

    E. Certificate for Associations

        Application for the certificates regarding the association registration, the leading cadre composition, and etc., could be made by completing the application form, or by submitting a letter affixed with the representative’s signature according to the statute of the association. l Applicable to BIR holders

l Please submit one photocopy of the applicant's BIR and one 1.5” recent colour photo.
